Marwa Elnahas: 4Ts Scoring Tool for Heparin – Induced Thrombocytopenia

Marwa Elnahas, Senior Neonatal and Pediatric Clinical Pharmacist, Clinical pharmacy trainer at Ministry of Health and Population – Egypt, shared a post on LinkedIn:

“4Ts scoring tool for heparin-induced thrombocytopenia (HIT)

A validated, widely used clinical pretest probability tool.

It helps the physician whether to order urgent diagnostic lab tests and whether to switch to alternative anticoagulation, thereby reducing overtesting and overdiagnosis.

4Ts stands for:

  1. ‘T’ hrombocytopenia (Magnitude) of The severity.
  2. ‘T’ iming of Platelet Count Fall.
  3. ‘T’ hrombosis.
  4. ‘T’ hrombocytopenia: Whether other reasons for a low platelet count are apparent.

Interpretation:

  • Low Risk (0 – 3 points): HIT is unlikely (lower 2% risk).
  • Intermediate Risk (4 – 5 points): Intermediate probability.
  • High Risk (6 – 8 points): High probability (~64% risk)”Marwa Elnahas
