Nour Al-Mozain: Why Stem Cell Collection Parameters Need Adjustment in SCD for Gene Therapy?

Nour Al-Mozain, Hemopathologist, Consultant at King Faisal Specialist Hospital and Research Center, shared a post on LinkedIn about a recent article by Yvette C Tanhehco et al., published in Blood Advances, adding:

“Stay Steady. Establish the Interface

Why do we need to adjust the standard parameters when collecting stem cells from SCD patients for gene therapy?

  1. Activated platelets and leukocytes promote aggregation and interface disruption
  2. Increased blood viscosity causes impaired component separation
  3. Abnormal RBC morphology leads to altered sedimentation dynamics

SCD blood exhibits elevated viscous resistance and abnormal flow behavior, even outside crisis states which leads to unstable interface and lower collection efficiency.

Performing RBC exchange immediately before collection may transiently reduce cellular activation.

Adjusting machine configuration by:
Increasing packing factor (centrifugal force) to compensate for increased viscosity

Collecting deeper within the buffy coat (accepting higher product Hct) improves CD34 and CE”
