“Is the Bone Marrow Examination still the ‘Gold Standard’?
Despite major advances in molecular diagnostics and imaging, bone marrow aspiration and trephine biopsy remain cornerstones in the hematologist’s diagnostic toolkit.
In practice, the challenge is not the procedure itself, but selecting the right patients and interpreting results in the correct clinical context.
Key clinical insights:
- Aspiration alone may be insufficient, particularly in fibrosis or lymphoma involvement.
- Trephine biopsy provides critical architectural detail often missed on aspirate.
- Essential in evaluating unexplained cytopenias, suspected leukemia, MDS, and marrow infiltration.
- Still a cornerstone for diagnosis, staging, and treatment decisions.
