“Streptokinase: The Clot Buster That Buys Time in Critical Moments
When minutes matter… reperfusion saves myocardium.
What is Streptokinase?
A fibrinolytic (thrombolytic) agent derived from Streptococci that activates plasminogen – plasmin, leading to clot breakdown.
Clinical Scenario
A 58-year-old male presents with:
- Crushing chest pain
- ST-elevation on ECG
- No PCI facility available
Streptokinase becomes a lifesaving option
Mechanism of Action
Streptokinase forms a complex with plasminogen:
- Converts more plasminogen to plasmin
- Degrades fibrin clot
- Restores blood flow
Indications
- STEMI (when PCI unavailable)
- Massive Pulmonary Embolism
- Deep Vein Thrombosis (selected cases)
- Arterial thrombosis
Contraindications (HIGH-YIELD)
- Active internal bleeding
- Recent surgery/trauma
- Hemorrhagic stroke history
- Severe uncontrolled hypertension
Adverse Effects
- Bleeding (most serious)
- Hypersensitivity reactions (foreign protein!)
- Hypotension during infusion
Clinical Pearl
- Prior streptococcal infection or previous use = antibody formation
- Reduced efficacy + ↑ risk of allergic reactions
Why not always Streptokinase?
Because newer agents (e.g., Alteplase) are:
- More fibrin-specific
- Faster acting
- Less antigenic
But In resource-limited settings, Streptokinase still saves lives daily
Bottom Line
Streptokinase may be old…
But in the right setting, at the right time – It’s the difference between life and death
