Alexis Guédon: Cancer-Associated Upper-Extremity Deep Vein Thrombosis
Alexis Guédon, Junior Deputy Editor at the European Journal of Preventive Cardiology, shared on LinkedIn about a recent article he and his colleagues co-authored, published in JACC Journals, adding:
“Upper-extremity deep vein thrombosis in cancer is sometimes viewed primarily as a catheter-related complication with a relatively low risk of recurrence.
In JACC Journals: CardioOncology, we report findings from 5,195 patients enrolled in the international RIETE registry, including 2,210 with active cancer.
We found that:
- Cancer was associated with higher risks of recurrent venous thromboembolism and bleeding.
- Risk differed substantially by cancer site: lung cancer had the highest recurrence risk, whereas genitourinary cancers had the highest bleeding risk.
- After anticoagulation discontinuation, the 2-year recurrence risk reached 17.2% in patients with lung cancer.
- Longer anticoagulation was associated with a lower risk of recurrence after treatment discontinuation, although these observational association should not be interpreted as causal.
These results suggest that cancer-associated upper-extremity deep vein thrombosis is more than a mechanical catheter-related complication.
Tumor biology appears to influence the balance between thrombosis and bleeding, supporting careful reassessment before anticoagulation is discontinued.
