Rainer Kaiser: How Neutrophils Stiffen Thrombi in Acute Ischemic Stroke

The clot stiffens

Exciting new concept in thromboinflammation on the cover of this week’s issue of Blood Journals Portfolio:

Zang et al. find that in acute ischemic stroke, thrombus-invading neutrophils contribute to thrombus stiffening through a SERPINB3-TGFbeta-collagen axis, which reshapes thrombus architecture and impairs post-stroke neurological recovery. Targeting SERPINB3 improves neurological recovery through reducing thrombus size and stiffness.

In patients, Zang et al. find that SERPINB3 levels correlate with response to systemic thrombolytic therapy and potently predict post-stroke neurological outcome in independent clinical cohorts, identifying it as novel biomarker and possible guidance for clinical decision-making.

Overall, this study reframes neutrophils as active mechanoregulators of arterial thrombosis in vivo and emphasizes the concept of thrombi ‘as living, evolving structures shaped by immune cells’, as my PhD student Maité Mulkers writes in the editorial covering this exciting work.”
