Andrea Bisso: Exploring the T Cell Engagers in Autoimmune Diseases

Andrea Bisso, Senior Director of Scientific Alliances – Head of Europe, shared a post on Linkedin about a recent article by Ricardo Grieshaber-Bouyer, published in Nature Reviews Immunology, adding:

“T cell engagers in autoimmune diseases

Despite advances in immunology, achieving durable, drug-free remission in refractory autoimmune diseases remains elusive due to incomplete B cell depletion
in tissue sanctuaries.

This perspective highlights how T cell engagers – off-the-shelf, bispecific antibodies – harness potent T cell-dependent cellular cytotoxicity to achieve deep B cell eradication and an unprecedented immune reset.

Take-home messages

  • TCEs redirect endogenous T cells to eliminate autoreactive B cells through T cell-dependent cellular cytotoxicity, combining CAR T-like potency with the convenience of an off-the-shelf biologic.
  • More than 30 TCEs targeting CD19, CD20 and B cell maturation antigen (BCMA) are already in clinical development across systemic lupus erythematosus, rheumatoid arthritis, systemic sclerosis, myasthenia gravis and other autoimmune diseases.
  • Next-generation engineering focuses on improving the therapeutic index through optimized CD3 affinity, multispecific designs, conditional activation and subcutaneous administration.

Why this matters

  • TCEs could shift treatment from long-term immune suppression to immune reset, while remaining repeatable, scalable and immediately available.

A Key challenges

  • Can efficacy be maintained while further reducing cytokine release syndrome?
  • Which B-cell target delivers the deepest and most durable remission?
  • Which patients require complete immune reset versus controlled immune modulation?”
