”Embolic stroke remains one of the most devastating complications of rheumatic heart disease, yet timely recognition and evidence-based management can significantly improve outcomes.

I created this infographic to provide a concise, practical overview of the complete management pathway—from identifying the cardioembolic source and acute reperfusion strategies to anticoagulation timing, valve intervention, and long-term secondary prevention.

Key learning points:

Reperfuse eligible patients without delaying evidence-based acute stroke care.

Avoid routine urgent therapeutic heparin after acute cardioembolic ischemic stroke.

Individualize the timing of anticoagulation based on infarct size, hemorrhagic transformation, and repeat neuroimaging.

Warfarin remains the standard of care for atrial fibrillation associated with moderate-to-severe rheumatic mitral stenosis, supported by the INVICTUS trial and contemporary international guidelines.
