Alejandro González Veliz: The Future of Cardiogenic Shock Lies in Precision Medicine

Alejandro González Veliz, Interventional Cardiologist at Institute of Cardiology and Cardiovascular Surgery, shared a post on LinkedIn about a recent article by Shashank Shekhar et al., published in Journal of Cardiothoracic and Vascular Anesthesia, adding:

“Cardiogenic shock remains one of the greatest challenges in cardiovascular medicine, with mortality still reaching 30–60% despite advances in reperfusion and mechanical circulatory support.

This updated review highlights a major shift in our approach: moving beyond simply treating hypotension toward recognizing shock early, using structured SCAI staging, invasive hemodynamic assessment, and multidisciplinary shock teams to personalize therapy.

Mechanical circulatory support is not a universal solution – timing, patient selection, and continuous reassessment are what truly determine outcomes.

As our understanding of shock physiology evolves, the future lies in precision medicine: identifying the right patient, selecting the right device, and intervening before irreversible organ injury occurs.

Which aspect of cardiogenic shock management do you think will have the greatest impact over the next decade: earlier diagnosis, invasive hemodynamic monitoring, or newer mechanical circulatory support strategies?”
