Rick Matthews: Cancer-Associated VTE Mortality and the Need for Coordinated Care

Rick Matthews, Sr. Vascular Consultant at Argon Medical Devices, Inc, Radiology Instructor at Pima Medical Institute, shared a post on LinkedIn about a recent article by Gang Li et al, published in Journal of Thrombosis and Thrombolysis, adding:

“This week’s VTE literature reinforces an important point: effective VTE care extends well beyond the procedure.

A new analysis of more than 312,000 VTE-related deaths among U.S. adults with cancer found that the age-adjusted mortality rate increased by 93.4 percent between 1999 and 2024.

Although the study does not evaluate thrombectomy or establish causation, it highlights the need for stronger oncology-focused VTE pathways—including early recognition, standardized risk assessment, appropriate anticoagulation, timely referral, and structured follow-up.

A second publication focused on IVC filter selection, complications, removal, and the risks associated with prolonged dwell time.

One of the most practical takeaways is that filter placement should be treated as part of a longitudinal care pathway—not as an isolated procedure.

Every filter pathway should clearly identify:

  • Why the filter was placed
  • Whether the anticoagulation contraindication is temporary or permanent
  • Who owns follow-up
  • When the patient will be reassessed
  • Whether and when retrieval should occur

The broader lesson is clear: successful VTE programs require coordinated patient identification, thoughtful treatment selection, documented follow-up ownership, and meaningful outcome measurement.

Technology matters—but the pathway surrounding that technology ultimately determines whether patients receive timely, coordinated care.”
