Anton Farkas: Mechanical Thrombectomy for Acute PE in Early Pregnancy

“Mechanical thrombectomy for acute pulmonary embolism in the first trimester of pregnancy.
We recently treated a 30-year-old woman at 10 weeks of pregnancy presenting with acute pulmonary embolism and a hemodynamic profile requiring urgent reperfusion therapy.
Following multidisciplinary assessment, we proceeded with catheter-based mechanical thrombectomy using Penumbra, Inc. Flash catheter, with particular emphasis on minimizing radiation exposure and meticulous fetal shielding throughout the procedure.
A substantial thrombotic burden was removed, including a more than 25 cm long cast-shaped venous thrombus, with subsequent improvement in the patient’s hemodynamic status.
Obstetric assessment performed the following day confirmed a vital fetus.
Acute pulmonary embolism during early pregnancy represents a rare and particularly challenging clinical scenario.
The therapeutic strategy must balance the urgency of maternal reperfusion with procedural safety and minimization of fetal risk. In selected patients, catheter-based mechanical thrombectomy may provide an important treatment option when rapid reduction of thrombotic burden is required.
This case also highlights the importance of multidisciplinary cooperation across specialties when treating complex and time-sensitive conditions.
