Michael Miyamoto: Scaling Guideline Based Lipid Management in Community Practice
Michael Miyamoto, Cardiologist – Mission Heritage Medical Group at Providence, share a post on LinkedIn about a recent article he and his colleagues co-authored, published in Journal of Clinical Lipidology, adding:
“Can complex lipid disorders be managed in community medical settings at scale?
Our new study in the Journal of Clinical Lipidology says yes and shows how we can close the gap between validated lipid guidelines and real world delivery.
Cohort (n equals 183, 2022 to 2024): Mean age 63, 62 percent female. 50 percent statin intolerant, 50 percent treatment naïve, 61 percent with ASCVD or DM or HTN. Mean baseline LDL C 154 mg dL.
Results (mean 9.4 mo follow up):
- LDL C low 49 mg dL (24.5 percent, p less than 0.001)
- Severe hypercholesterolemia: 31 percent lead to 6 percent of cohort
- Improvements held across ASCVD, DM/HTN, elevated Lp(a), and hypertriglyceridemia
Combination and nonstatin therapy drove the gains:
- Combination therapy use roughly doubled (15.9 percent lead to 33.5 percent, p less than 0.001), with a clear dose response: escalation to more agents tracked with the largest LDL C drops
- For statin intolerant patients, liberal use of PCSK9i mAbs, ezetimibe, and bempedoic acid delivered substantial LDL C reductions, with 10 percent ultimately tolerating a statin
- Most frequent 2 drug regimens included combinations of statin PCSK9i ezetimibe
Genetic testing found actionable risk: 20 percent of those tested (21 or 105) had pathogenic FH variants response was strongest in this group (LDL C low 36.3 percent), with cascade testing counseling extended to families.
The takeaway: A lean team 2 cardiologists, a genetic counselor, a dietitian, a pharmacy tech, each at less than 10 percent time operationalized existing guidelines and expanded therapeutic options within a community practice. No new funding required.
A replicable model for bringing proven lipid management to patients at scale.”
