James Fildes: Modeling Device Related Thrombosis in Multi Organ Systems

James Fildes, Chief Executive Officer and Chief Scientific Officer at Pebble Biotechnology Laboratories, shared a post on LinkedIn:

”When blood touches a foreign surface, plasma proteins coat it within seconds and platelets begin to settle onto that coating.

This is why thrombosis is a major problem for every device that removes blood from a patient and puts it back.

That includes bypass, ECMO, dialysis and ventricular assist devices, and the tubing, cannulae and stents that connect them to the patient.

Thrombogenicity is currently assessed by circulating blood through plastic loops with no organs present.

The difficulty is that devices rarely fail in the tubing itself.

They fail downstream in the patient’s microvasculature, which a plastic loop doesn’t possess.

Our multi-organ systems do, which is why we’ve started using them to model it.

We withdraw anticoagulation and let the vascular beds thrombose while we watch in real time: mean arterial and venous pressures climb and flow falls as individual arteries occlude.

It’s a disconcerting process to watch circulation rapidly fail, but seeing where and when it happens in a living vascular bed is something a plastic loop cannot show, and something that would otherwise only be learned from an animal or a patient.”
