Orly Leiva: Rethinking JAK Inhibitor Cardiovascular Safety in MPNs
Orly Leiva, Cardio-Oncology and Pulmonary Hypertension Cardiologist, Assistant Professor at University of Kentucky College of Medicine, shared a post on LinkedIn about a recent article he and his colleagues co-authored, published in JACC: CardioOncology, adding:
“Excited to share out new publication on the impact of JAK Inhibitors on cardiovascular outcomes in hematologic malignancies, particularly MPNs!
In this JACC Cardio-Oncology primer, we looked at the literature on cardiovascular risk with JAK inhibitors.
On the basis of the ORAL Surveillance trial, which demonstrated increased cardiovascular risk in rheumatoid arthritis with JAK Inhibitor vs TNF inhibitors, the FDA has placed a black box warning on all JAK inhibitors.
However, recent data from our group and others suggest that the impact of JAK inhibition is more nuanced.
Among patients with MPNs, for which JAK inhibitors are sometimes indicated in, suggest that there may be a reduced risk of thrombosis and HF events with JAK inhibition.
The discordance between the literature in RA and other autoimmune diseases and MPNs may be due to underlying disease pathophysiology, specific JAK-STAT types targeted and patient population.
More work needs to be done!
