D Chandra: Reduce the Apixaban Dose for a Patient with VTE and a Creatinine Clearance of 25?
D Chandra, Consultant Haematologist and Director, Staffordshire Thrombosis and Anticoagulation Centre at University Hospitals of North Midlands NHS Trust, shared a post on LinkedIn:
“Reduce the apixaban dose for a patient with VTE and a creatinine clearance of 25?
Many prescribers would, but the dose-reduction rules for AF don’t carry over to VTE treatment.
The details of DOAC prescribing are where errors happen: dose by indication, renal function, interactions, bleeding, and what to do before a procedure.
So I’ve made a free course on HaemAcademy: seven short animated videos, under 40 minutes in total, from choosing the right patient through to managing major bleeding and planning surgery.
Each part has five questions, with on-screen references to the guidance behind every recommendation.
There’s a final assessment and a certificate of completion.
For doctors, pharmacists, nurses and trainees who prescribe or manage anticoagulation.”
