Rana Al-Hrout: Approach to Thrombocytopenia – A Practical Diagnostic Roadmap

Rana Al-Hrout, Clinical Pathology-Hematology Specialist at Ministry of Health – Jordan, shared a post on LinkedIn:

“Approach to Thrombocytopenia: A Practical Diagnostic Roadmap

Thrombocytopenia is a common laboratory finding, but identifying its underlying cause requires a systematic approach.

The differential diagnosis ranges from pseudothrombocytopenia and peripheral platelet destruction to increased consumption and complex bone marrow disorders.

A practical diagnostic approach includes:

Step 1. Confirm True Thrombocytopenia

Review the peripheral blood film for platelet clumping and repeat the CBC using citrate when indicated.

Step 2. Assess the Clinical Context

Consider acute versus chronic onset, bleeding history, medications, recent infections, pregnancy, liver disease, and autoimmune conditions.

Step 3. Determine the Hematological Pattern

Is thrombocytopenia isolated, or is it associated with anemia and/or leukopenia?

Step 4. Identify the Underlying Mechanism

  • Decreased platelet production
  • Increased platelet destruction
  • Increased platelet consumption
  • Splenic sequestration
  • Dilutional thrombocytopenia

Key Takeaway

Always confirm the platelet count, evaluate the peripheral blood film, assess the other blood cell lines, and identify the underlying mechanism before reaching a diagnosis.”
