Ravindra Prasad Thokala: Stem Cell Apheresis – 4 Donors, 4 Unique Challenges
Ravindra Prasad Thokala, Transfusion Medicine Consultant and Head at Apollo Cancer Center, shared a post on LinkedIn:
“All in a day ( last week in a single day , though a regular happening )at the Stem Cell Advanced Apheresis Unit.
Four stem cell collections, four very different donors, one day:
A Matched Unrelated Donor (MUD) with body weight significantly lower … for a high-body-weight recipient, where reaching the CD34+ target meant careful planning of processed blood volume, the time spent on the machine and addressing the donor needs and comfort .
- A fully matched sibling donor
- A haploidentical mother, giving her child a second chance
- A low-weight child ..autologous PBSC collection in a heavily pre-treated patient, where every millilitre of extracorporeal volume, every degree of warmth and every minute on the machine counts.
Each procedure demands strict traceability at every step: equipment IDs, consumable lot numbers, anticoagulant and fluid batches, and a Precise labelling.
Every bag must link back to its donor, its recipient, its operator and its process without a single gap. In cellular therapy, the product label carries the full history of how the product was made, not just a name.
Days like this feel complete and deeply satisfying, especially when every collection target is met. That matters most for a high-weight MUD recipient and a heavily treated autologous patient, where there is little margin for a second attempt.
None of this happens alone. A trained, efficient team where everyone contributes equally, from apheresis nurses and technologists to physician assistants ,is what makes complex days like this safe and successful.
Thankful for my team. Grateful to every donor.”
