”Isolated distal DVT isn’t always ‘minor VTE.’

The real question is often:

Anticoagulate… or monitor with serial ultrasound?

My quick framework:

Treat (often 3 months) if:

  • significant symptoms
  • extensive clot / close to proximal veins
  • high risk of extension (unprovoked, cancer, prior VTE, immobility)

Observe if:

  • low-risk features
  • higher bleeding risk
  • reliable follow-up is guaranteed

Both strategies can be correct.”
