“One of the major roles of Clinical pharmacist is to check Drug-Drug and Drug-Diluent incompatibilities especially for patients in ICUs and hematology units as they are considered a high-risk group for the occurrence of incompatibilities because they commonly require the use of multiple drugs, most of which are administered intravenously and by continuous infusion.

Potential complications of co-administration of incompatible drugs include:

  • Precipitation
  • Central venous catheter (CVC) occlusion or malfunction
  • Reduced potency of medication
  • Embolism and local or systemic inflammatory reactions.”
