“One of the fundamentals of hematology is recognizing how a neutrophil develops from the myeloblast to the mature segmented neutrophil.
Each maturation stage brings important changes in nuclear shape, chromatin, cytoplasm, and granules that help us identify cells accurately under the microscope.
Things to know:
- Myeloblast – large nucleus, fine chromatin, scant basophilic cytoplasm; no granules
- Promyelocyte – primary (azurophilic) granules appear
- Myelocyte – specific/secondary granules begin; nucleus starts indenting
- Metamyelocyte – characteristic kidney-shaped nucleus
- Band neutrophil – U-shaped/band nucleus without segmentation
- Segmented neutrophil – mature cell with a 3-5 lobed nucleus
As neutrophils mature, the nucleus becomes more condensed and progressively indented/segmented, while secondary granules increase.
Understanding these morphological changes is essential for accurate CBC differential interpretation, peripheral smear review, and recognizing a left shift.”
