Christabel Odeta: Treating Pediatric Cancer in Children With Sickle Cell Disease

A Child With Cancer and Sickle Cell Disease Is a Third Species of Patient.
Standard pediatric oncology protocols assume a marrow that can be pushed that recovers between cycles, tolerates myelosuppression, metabolizes predictably. Sickle cell disease violates every assumption simultaneously: chronic hemolysis, hyperplastic marrow, endothelial dysfunction, stroke history, prior alloimmunization.
Layer on malignancy and the standard protocols stop being treatment plans and become hazard lists.
The dual pathway navigator exists because this intersection requires independent therapeutic logic, not adaptation.
The pharmacogenomic hard stops are where independent logic begins: TPMT deficiency forces 6 mercaptopurine reduction or substitution in compromised marrow, a standard dose is an overdose; UGT1A1 governs irinotecan; codeine is contraindicated in all children, and in sickle cell variable CYP2D6 metabolism makes the contraindication absolute; G6PD status gates rasburicase before tumor lysis therapy. G6PD status gates rasburicase before tumor lysis therapy.
Tumor lysis syndrome is the acute lethality of pediatric oncology: Cairo Bishop criteria enforcement, aggressive hydration, electrolyte surveillance with telemetry, allopurinol standard rasburicase only with validated G6PD, because rasburicase in a deficient patient produces hemolysis on top of hemolysis.
Neurovascular protection runs through treatment: chronic transfusion support to maintain stroke prevention, TCD surveillance, exchange transfusion escalation on risk triggers because hypoxia, dehydration, infection, and anemia, all common in cytotoxic therapy, are precisely the vaso occlusive triggers. Blood product governance is non negotiable: irradiated, CMV negative, extended phenotype matched units alloimmunization from chronic transfusion means a mismatched unit today is an incompatible transfusion and a future transplant obstacle tomorrow.
Fertility preservation is planned before the first cycle: post pubertal sperm banking; prepubertal ovarian tissue cryopreservation as the documented experimental pathway; GnRH agonist protection where appropriate.
The psychosocial architecture carries what physiology cannot: school continuity, developmental support, family burden assessment, longitudinal trauma surveillance because a child navigating two life threatening diseases simultaneously is carrying an adults load on a childs frame.
P98 protects curative intent and physiologic survival in the same protocol the only honest way to treat the third species.”
