Dinesh Viruvanti: Ineffective Erythropoiesis is not Just ‘Reduced RBC Production’

Dinesh Viruvanti, Pediatric Hematologist and Oncologist at Yashoda Hospitals, shared a post on LinkedIn:

“Ineffective erythropoiesis is not just ‘reduced RBC production’.

It is a high – effort marrow state with poor effective RBC output – often due to intramedullary hemolysis.

This distinction is clinically useful:

Peripheral hemolysis usually gives a good reticulocyte response, while ineffective erythropoiesis gives anemia with biochemical hemolysis but an inadequate reticulocyte response.

A concept worth revisiting in children with thalassemia, megaloblastic anemia, congenital dyserythropoietic anemia, sideroblastic anemia, and marrow disorders.”
