“A recent study published in JAMA Network Open highlights an important clinical message:
The presence of atrial fibrillation and a high CHA₂DS₂-VASc score alone may not be sufficient to establish the net clinical benefit of anticoagulation in older adults with severe functional disability.
Anticoagulation decisions should go beyond conventional risk scores, incorporating frailty, life expectancy, bleeding risk, goals of care, and, importantly, patient preferences.
The key question is not simply whether a patient is at high risk of stroke, but whether anticoagulation offers a meaningful net benefit in the context of their overall health and care priorities.”
