“Case study: The APTT was 96 seconds.
The patient had no history of bleeding.
So what would You do next?
A 72-year-old patient arrives with:
Extensive spontaneous bruising, with painful muscle haematomas.
There is no previous bleeding disorder and not on anticoagulant.
The coagulation screen shows:
- PT: Normal
- APTT: 96 sec (significantly prolonged)
Now here’s the question for you
- What’s your First thought?
- Heparin contamination?
- Lupus anticoagulant?
- Factor deficiency?
- Acquired inhibitor?
This is where coagulation becomes detective work.
