Jin-Xiong She: Functional Iron Deficiency in the Setting of Chronic Inflammation

“Iron deficiency isn’t always about how much iron you have.
Sometimes it’s about where your body is hiding it.
Iron does more than build red blood cells.
Inside cells, it’s essential for mitochondrial heme production and the electron transport chain itself.
Chronic inflammation disrupts that supply in a specific way.
- Inflammatory signals raise a hormone called hepcidin.
- Hepcidin locks iron inside macrophages and the liver.
- That iron becomes unavailable to tissues that need it, even though total body iron is fine.
This is called functional iron deficiency.
Hemoglobin can stay completely normal while cells throughout the body are quietly starved of iron.
Mitochondria feel this first.
- Heme synthesis slows down
- Electron transport chain function weakens
- Oxidative capacity in muscle and brain drops
The result rarely looks like textbook anemia.
It looks like fatigue, brain fog, and exercise intolerance that standard bloodwork can’t explain.
This pattern shows up often in chronic inflammatory conditions, from autoimmune disease to kidney disease.
It likely shows up more broadly than most testing ever catches.
Iron isn’t the problem when this happens.
