Here’s a simple clinical approach 

Iron deficiency anemia

  • Decreased Serum iron
  • Increased TIBC
  • Decreased Ferritin

Anemia of chronic disease

  • Decreased Serum iron
  • Decreased / normal TIBC
  • Increased Ferritin

Iron is present but not available (hepcidin effect)

Iron overload

  • Increased Serum iron
  • Decreased TIBC
  • Increased Ferritin

Risk of iron deposition – liver, heart, endocrine organs

Clinical tip:

  • Ferritin is a storage marker and an acute phase reactant

Always interpret it in clinical context
