Understanding iron metabolism is the key.
Here’s a simple clinical approach
Iron deficiency anemia
- Decreased Serum iron
- Increased TIBC
- Decreased Ferritin
Anemia of chronic disease
- Decreased Serum iron
- Decreased / normal TIBC
- Increased Ferritin
Iron is present but not available (hepcidin effect)
Iron overload
- Increased Serum iron
- Decreased TIBC
- Increased Ferritin
Risk of iron deposition – liver, heart, endocrine organs
Clinical tip:
- Ferritin is a storage marker and an acute phase reactant
Always interpret it in clinical context
