Nasrin Samadi: Can a Normal Clotting Test Be Misleading?

Nasrin Samadi, Laboratory Founder and Owner at Mehr laboratory, shared a post on LinkedIn:

“Can a Normal Clotting Test Be Misleading?

A normal PT or aPTT result does not always mean that anticoagulant activity is absent. Similarly, a prolonged aPTT does not necessarily indicate an increased risk of bleeding.

In Episode 3 of Medical Insights with Dr. Nasrin Samadi, we explore some important challenges in interpreting coagulation tests:

  • Preanalytical errors: How underfilled citrate tubes or heparin contamination can affect coagulation test results.
  • Lupus anticoagulant: Why a prolonged aPTT may be associated with an increased risk of thrombosis rather than bleeding.
  • DOACs: Why normal PT and aPTT results cannot reliably exclude clinically significant anticoagulant effects.
  • Specialized testing: When drug-specific anti-Xa assays or other specialized coagulation tests may be helpful.

The key message:

Laboratory results should never be interpreted in isolation. Understanding the patient’s clinical context is just as important as understanding the numbers.
