Parth Parmar: RBC Morphology – A Practical Guide to Blood Smear Interpretation

Parth Parmar, Attending Physician at District hospital, Gandhidham, shared a post on LinkedIn:

Topic 34:

The red blood cell morphology section of the peripheral blood smear is one of the highest-yield components of hematology.

When interpreted systematically, it provides invaluable diagnostic clues for anemia, hemolytic disorders, hemoglobinopathies, marrow diseases, and systemic illnesses.

Our RBC morphology master guide is organized into three practical parts:

  • Part 1 – RBC size and colour abnormalities: Understanding microcytosis, macrocytosis, hypochromia, anisocytosis, and correlation with RBC indices (MCV, MCH, MCHC, RDW).
  • Part 2 – RBC shape abnormalities: Recognizing spherocytes, schistocytes, target cells, sickle cells, elliptocytes, dacrocytes, acanthocytes, echinocytes, stomatocytes, and their clinical significance.
  • Part 3 – Pattern recognition master guide: Integrating RBC morphology with CBC indices and clinical context to rapidly narrow the differential diagnosis through structured pattern recognition.

The goal is not simply to identify abnormal cells – but to understand the pathological process they represent and translate smear findings into meaningful clinical decisions.

Which RBC morphology pattern has challenged you the most in day-to-day practice?”
