Rupam Manna: Not All Portal Vein Involvement in HCC Is the Same

“Not all portal vein involvement in HCC is the same.

The Vp classification helps define the extent of portal vein tumor thrombosis (PVTT) – one of the most important factors influencing prognosis and treatment strategy in hepatocellular carcinoma.

  • Vp0–Vp2: Early vascular involvement where selected patients may still benefit from surgery or locoregional therapies.
  • Vp3–Vp4: Major portal vein invasion, usually requiring systemic therapy, with Y-90 radioembolization (TARE) emerging as an important option in carefully selected patients.

Understanding the anatomy of portal vein invasion is key to making the right therapeutic decision.
