Salih Ehsan: Diagnostic Screening Strategy for Hemoglobinopathy Traits
Salih Ehsan, Chief Executive Officer at Kulsoom Society of Hematology, shared a post on LinkedIn:
“To pick 6 most common Hemoglobinopathies traits use these steps
- Family history of transfusion depends anemia
- Ethnicity
- CBC ( Erythrocytosis, mild anemia with moderate microcytosis, normal RDW cv with low RDW SD)
- Normal iron studies
- HPLC finding ( HbA2 and HbE can co-elute , if HbA2 is above 9% suspect HbE, and HbS and HbD can give common peak , solubility test will help to differentiate between HbS and HbD trait)
- Peripheral smear, in alpha and beta Thalassemia trait few target cells, while HbE and HbC have mild to moderate target cells. In HbS and HbD trait peripheral smear not helpful.”
