Sally Mandour: Platelet Size And Shape Can Reveal The Diagnosis

Sally Mandour, Associate Professor, Head of Hematology Unit at Clinical Pathology Department of National Liver Institute, Monofeya University, shared a post on LinkedIn:

”Platelet’s shape and size:

Platelet morphology evaluation is one of those quietly powerful tools in hematology — it doesn’t get the same spotlight as platelet counts, but it can point straight to a diagnosis.

Normal platelets are 1–4 μm, round or oval, with granules spread evenly through the cytoplasm.

Abnormal findings can be:

  • Occasional giant/large platelets → often seen in destructive conditions (ITP) or MPNs (like myelofibrosis) and here platelets count is the clue

Other important hereditary disorders are:

  • Microplatelets as in X-linked thrombocytopenia (a)
  • Many giant platelets; a Döhle body-like inclusion as in MYH disease(b)
  • Gray platelet syndrome with platelets lacking alpha granules (c)

Finally, Don’t skip the platelet indices.

Morphology under the microscope tells only half the story — platelet indices like MPV (mean platelet volume), PDW (platelet distribution width), and P-LCR (platelet large cell ratio) give the quantitative backbone to that visual impression.”
