Anticoagulation therapy plays a vital role in preventing and managing thromboembolic disorders across medical and surgical settings. From atrial fibrillation to post-operative care, the safe and effective use of anticoagulants requires careful clinical judgment and monitoring.
Key Anticoagulation Medications:
• Heparin & Low-Molecular-Weight Heparins (Enoxaparin)
• Vitamin K Antagonists (Warfarin)
• Direct Oral Anticoagulants (DOACs): Apixaban, Rivaroxaban, Dabigatran, Edoxaban
Clinical Focus Areas:
• Individualized dosing based on renal function and patient risk
• Monitoring for bleeding and thrombotic complications
• Managing drug–drug and drug–food interactions
• Peri-operative anticoagulation planning
