”Ferritin after IV iron: Timing matters
Seeing a ferritin of 800 µg/L two weeks post-infusion? Don’t panic – and definitely don’t start chelation.
Ferritin is an acute-phase reactant. After IV iron, it rises transiently for days to weeks, reflecting repletion, not overload.
Best practice:
Check Hb at 2–4 weeks (primary endpoint)
Defer ferritin + TSAT until ≥4–8 weeks
Use TSAT for early iron status assessment
Iron overload requires persistent ferritin >1000 µg/L, organ iron on MRI, and predisposing conditions – rare outside transfusion-dependent states.
Misinterpreting post-infusion ferritin leads to unnecessary anxiety and inappropriate referrals. Patience and context prevent harm.”
