Johanna Gebhart: New Publication from the Vienna Bleeding Biobank Advancing BDUC Research Is Now Live

”Bleeding disorder of unknown cause: is impaired thrombin generation part of the answer?
Bleeding disorder of unknown cause (BDUC) is a diagnosis given to patients with a clear bleeding tendency but normal results on standard haemostatic tests.
Understanding the underlying mechanisms remains a major challenge.
Thrombin generation (TG) has been proposed as a more sensitive tool but previous studies have yielded conflicting results.
A new case-control study in RPTH Journal set out to reproduce earlier findings using a fully automated TG analyser at different tissue factor concentrations.
Key findings from 62 BDUC patients vs 22 healthy controls:
- Patients had significantly longer time to peak, lower peak thrombin, and lower velocity index with both trigger reagents
- Area under the curve (ETP) was significantly lower with the very low tissue factor reagent
- ROC analysis showed moderate discriminatory performance
However, no significant association was found between TG parameters and bleeding score after adjustment for FVIII and FIX activity
This study reproduces previous findings of impaired TG in BDUC, adding to the evidence that subtle coagulation dysfunction may underlie bleeding in this understudied population.”
