Mark Sheppard, Assistant Deputy Director of Pharmacy at Monash Health, shared a post on LinkedIn about a recent article he and his colleges co-authored, published in Journal of Thrombosis and Thrombolysis, adding:
“The Anticoagulation Stewardship Service at Monash Health is a strong example of how multidisciplinary collaboration, combined with data, can deliver meaningful improvements in patient safety.
Pleased to have contributed to the team’s recent publication, which outlines a scalable approach integrating a digital dashboard to support screening and prioritisation of at-risk patients for clinical review.
While applied here for anticoagulants, the model has broader relevance for medicines stewardship across health services.”
