Michael Wilkinson: How Lp(a) Shapes Coronary Plaque and Cardiovascular Risk

Michael Wilkinson, Associate Professor of Medicine at UC San Diego Health, shared a post on LinkedIn about a recent article he and his colleagues co-authored, published in Current Atherosclerosis Reports, adding:

“Our timely, state-of-the-art review now published in Springer Nature Current Atherosclerosis Reports:

‘The Evolving Role of Coronary Artery Imaging in Assessing Cardiovascular Risk from Lipoprotein(a).’

  • CAC scoring shows associations with Lp(a) incidence and progression, though findings across studies remain inconsistent.
  • CCTA offers the most detailed noninvasive assessment, linking elevated Lp(a) to greater plaque prevalence, multivessel disease, and high-risk features like low-attenuation plaque, positive remodeling, and napkin-ring sign.
  • Invasive imaging (OCT, IVUS) reinforces this, with elevated Lp(a) tied to thin-cap fibroatheroma, lipid-rich plaque, and greater atheroma progression over time.

There’s still a lot to learn about how Lp(a) uniquely shapes plaque biology and risk – this is a critical area for Lp(a) research in the coming years.”
