Shehab Mohamed: A Systematic Review on Bleeding as Initial Presentation of CML
Shehab Mohamed, Hematologist at Hamad Medical Corporation, shared on LinkedIn about a recent article by Mohammad ALTermanini et al, published in Clinical and Applied Thrombosis/Hemostasis, adding:
”Continuing our work in myeloid neoplasms and hemostasis
We are proud to share our latest progress in this evolving field.
After previously exploring:
- Bleeding in acute myeloid leukemia (AML)
- Thrombosis in acute promyelocytic leukemia (APL)
- We now expand our work with a new systematic review on bleeding as the initial presentation of chronic myeloid leukemia (CML).
Key findings from our review:
- 53 reported cases identified across the literature
- Strong male predominance (74%)
- Median age: 45 years
Central nervous system bleeding was the most common presentation (40%), followed by:
- Soft tissue hematomas (25%)
- Visceral/splenic bleeding (19%)
- Most patients presented with marked hyperleukocytosis
Despite severity:
- 62% survived
- 21% mortality, mainly due to intracranial hemorrhage or splenic rupture
Key message: Bleeding can be the first manifestation of CML – and should not be overlooked, especially in patients presenting with unexplained hemorrhage and extreme leukocytosis.
This work highlights:
- The complex interplay between malignancy and hemostasis
- The need for early recognition
- The importance of multidisciplinary management
We are excited to keep pushing forward in this field with our team – more to come.”
