“Deep Vein Thrombosis (DVT)
Definition: Deep Vein Thrombosis is the formation of a blood clot in a deep vein, usually in the lower limb, leading to partial or complete obstruction of blood flow.
Major complication: Pulmonary Embolism (clot travels to lungs).
Causes/Risk Factors (Virchow’s Triad)
- Venous stasis: Prolonged bed rest, immobility
- Endothelial injury: Surgery, trauma, fractures
- Hypercoagulability: Pregnancy, cancer, smoking, OCP use
Signs and Symptoms
- Unilateral leg swelling
- Calf pain or tenderness
- Warmth and redness
- Dilated superficial veins
Investigations
- D-dimer test
- Doppler ultrasound (most common)
- Venography (gold standard)
Medical Management
- Anticoagulants (Heparin, Warfarin)
- Compression stockings
- Thrombolytic therapy
- Early mobilization
- Physiotherapy Management
- Early ambulation (as advised)
- Ankle pump exercises
- Limb elevation
- Deep breathing exercises
- Compression therapy
Precaution: Avoid massage over affected limb.
Complications
- Pulmonary embolism
- Post-thrombotic syndrome
- Chronic venous insufficiency
