“Deep Vein Thrombosis (DVT)

Definition: Deep Vein Thrombosis is the formation of a blood clot in a deep vein, usually in the lower limb, leading to partial or complete obstruction of blood flow.

Major complication: Pulmonary Embolism (clot travels to lungs).

Causes/Risk Factors (Virchow’s Triad)

  • Venous stasis: Prolonged bed rest, immobility
  • Endothelial injury: Surgery, trauma, fractures
  • Hypercoagulability: Pregnancy, cancer, smoking, OCP use

Signs and Symptoms

  • Unilateral leg swelling
  • Calf pain or tenderness
  • Warmth and redness
  • Dilated superficial veins

Investigations

  • D-dimer test
  • Doppler ultrasound (most common)
  • Venography (gold standard)

Medical Management

  • Anticoagulants (Heparin, Warfarin)
  • Compression stockings
  • Thrombolytic therapy
  • Early mobilization
  • Physiotherapy Management
  • Early ambulation (as advised)
  • Ankle pump exercises
  • Limb elevation
  • Deep breathing exercises
  • Compression therapy

Precaution: Avoid massage over affected limb.

Complications

  • Pulmonary embolism
  • Post-thrombotic syndrome
  • Chronic venous insufficiency
