“Acute lower limb ischemia is one of the most feared diagnoses to miss in emergency medicine.

Ironically, it often isn’t missed because it’s subtle, it’s missed because we fail to think of it.

It is mistaken for radiculopathy, chronic pain, deep vein thrombosis, neuropathy, or musculoskeletal disease while the clock continues to tick.

Yet one of the most important diagnostic tests takes only seconds: palpate the pulses.

A high index of suspicion, a vascular examination, and timely recognition can mean the difference between limb salvage and amputation, and sometimes between life and death.

Despite having classic clinical features, acute limb ischemia continues to be overlooked, with devastating consequences.
