“As a nurse we should be very responsible when transfusion blood components to the patient body, because one mistake can form big issue to patient life.

Blood Components Transfusion – Nursing Alerts and Important Points

  • Patient Identification
  • Verify patient identity with two identifiers.
  • Match blood bag details with doctor’s order and patient records.

Before Transfusion

  • Obtain informed consent.
  • Check blood group and cross-match results.
  • Record baseline vital signs (T, P, R, BP, SpO₂).
  • Ensure patent IV access (18–20G preferred for adults).

During Transfusion

  • Start slowly for the first 15 minutes.
  • Stay with the patient during the initial period.
  • Monitor vital signs regularly.
  • Use only 0.9% Normal Saline with blood products.
  • Observe for signs of transfusion reaction.

Signs of Transfusion Reaction

  • Fever or chills
  • Rash or itching
  • Dyspnea (shortness of breath)
  • Chest or back pain
  • Hypotension or hypertension
  • Dark urine

If Reaction Occurs

  • Stop transfusion immediately.
  • Maintain IV line with Normal Saline.
  • Inform physician and blood bank.
  • Monitor vital signs closely.
  • Send blood bag and samples as per protocol.

Blood Component Specific Alerts

  • Packed Red Cells (PRBC): Monitor for fluid overload.
  • Platelets: Watch for allergic reactions and fever.
  • Fresh Frozen Plasma (FFP): Monitor for anaphylaxis and volume overload.
  • Cryoprecipitate: Observe for hypersensitivity reactions.
