Yubrine Moraa Gachemba: BETULA Adds New Evidence for Catheter-Directed Thrombolysis in PE

Pulmonary embolism care is entering a more nuanced era.

BETULA adds important randomized evidence that carefully selected patients with intermediate-high-risk PE may achieve earlier right ventricular unloading with low-dose, short-duration catheter-directed thrombolysis, without an observed increase in major bleeding.

But the larger question is not simply whether we can intervene earlier. It is which patients derive meaningful clinical benefit, at what risk, and within which health systems.

For Africa and the wider Global South, this matters enormously. Innovation in PE care must be accompanied by better risk stratification, multidisciplinary decision-making, appropriate imaging, and equitable access to reperfusion strategies.

The future of PE management will not be anticoagulation versus intervention. It will be precision in selecting the right patient, for the right therapy, at the right time.

“In the randomised BETULA trial, low-dose, short-duration catheter-directed thrombolysis improved early right ventricular unloading compared with anticoagulation alone in patients with intermediate-high-risk pulmonary embolism, without increasing major bleeding.”
