Irradiation of blood components: from clinical indication to laboratory release

Preventing transfusion-associated graft-versus-host disease (TA-GVHD) requires timely identification of patients who need irradiated cellular components. Irradiation prevents donor T-lymphocyte proliferation; leukoreduction does not provide equivalent protection.

I have prepared these two infographics as a practical reference for clinicians and transfusion laboratory teams.

The clinical poster addresses indications, component selection and situations in which irradiation is not routinely required. The laboratory poster covers dose requirements, process validation, quality control, traceability, storage restrictions and processing costs.

The material draws on AABB, British Society for Haematology, NHS Blood and Transplant, and WHO resources. American and European requirements are distinguished where they differ, particularly for dose specifications and post-irradiation red-cell storage.

In practice, the challenge extends beyond delivering the prescribed radiation dose. Treatment history must be available, irradiation requirements must remain visible across admissions, and the correct component must reach the patient within its revised expiry.

These summaries are intended to support clinical discussion, staff training and review of institutional irradiation policies. They should be used alongside locally approved protocols and component specifications.”
