”Isolated distal DVT isn’t always ‘minor VTE.’
The real question is often:
Anticoagulate… or monitor with serial ultrasound?
My quick framework:
Treat (often 3 months) if:
- significant symptoms
- extensive clot / close to proximal veins
- high risk of extension (unprovoked, cancer, prior VTE, immobility)
Observe if:
- low-risk features
- higher bleeding risk
- reliable follow-up is guaranteed
Both strategies can be correct.”
