Adil Yousaf: Thrombolysis Speed Matters, but Eligibility Comes First

“Thrombolysis is among the most time-critical treatments in medicine but speed never replaces eligibility screening.
Thrombolytics, also called fibrinolytics, accelerate conversion of plasminogen to plasmin, which breaks down fibrin within an existing clot.
They are not interchangeable:
- Alteplase (Activase or Actilyse) is a recombinant tissue plasminogen activator. US labeling includes acute ischemic stroke, acute myocardial infarction and selected acute massive pulmonary embolism.
- Tenecteplase (TNKase or Metalyse) is a modified tPA. Current US and EU authorizations include acute ischemic stroke and STEMI, but labeled time windows and product presentations differ. The 2026 AHA/ASA stroke guideline endorses either alteplase or tenecteplase for eligible patients in the 4.5-hour thrombolytic window.
- Reteplase (Retavase) has a US STEMI indication.
- Urokinase (Kinlytic) has US labeling for selected acute massive pulmonary embolism; current availability must be confirmed locally.
The essential safety pearl is catastrophic bleeding prevention. Before treatment, confirm the diagnosis and review brain imaging when stroke is suspected, blood pressure, bleeding history, recent surgery or trauma, antiplatelet and anticoagulant exposure, and relevant laboratory results.
