”A prediction: in a few years, managing major bleeding without understanding ROTEM will be comparable to practicing medicine without knowing how to read an ECG.
We are moving from fixed transfusion ratios such as 1:1:1 toward individualized, goal-directed hemostatic resuscitation.
ROTEM helps us answer the question that really matters:
What is this patient actually missing?
Fibrinogen?
Coagulation factors?
Platelet contribution?
Is there fibrinolysis?
The goal is no longer simply to transfuse blood products.
The goal is precision hemostasis.”
