“ACS is not one diagnosis – it’s a spectrum.
The most important distinction is simple:
Unstable angina – ischemia, no myocardial necrosis, troponin normal
NSTEMI – necrosis, troponin is high, no ST elevation
STEMI – necrosis and ST elevation
And remember: Low troponin doesn’t mean low risk.
ST elevation means act fast.
