Umeshkumar C.: Evaluating Scoring Systems and Biomarkers for DIC in Critically Ill Patients

“Evaluating Scoring Systems and Biomarkers for DIC in Critically Ill Patients
Could a simple scoring system help detect a life-threatening condition before it’s too late?
Disseminated Intravascular Coagulation (DIC) is one of the most challenging syndromes encountered in intensive care. It is not just a bleeding disorder – it is a complex process where abnormal clot formation and uncontrolled bleeding occur simultaneously, often leading to multi-organ failure if not recognized early.
In this comic episode, I simplify the science behind DIC using an engaging story while highlighting the key findings from a prospective clinical study.
Key clinical insights
- DIC commonly develops in sepsis, trauma, malignancy, liver disease, and obstetric emergencies.
- No single laboratory test can diagnose DIC accurately.
- Composite scoring systems improve diagnostic confidence and early recognition.
The JAAM scoring system demonstrated the strongest overall diagnostic performance for early detection when compared with other evaluated systems.
Important biomarkers include D-dimer, platelet count, PT, aPTT, fibrinogen, antithrombin III, and RDW.
Early identification can support timely intervention and improve outcomes in critically ill patients.
Why this matters:
Every hour counts in critical care. Combining clinical judgment with validated laboratory scoring systems can help clinicians identify DIC earlier, initiate appropriate management, and potentially save lives.”
