When iron supplementation isn’t fixing the anemia, it’s time to think bigger:
- Iron: Still the #1 deficiency after bariatric surgery.
- Copper: Deficiency can mimic iron deficiency with anemia + neutropenia.
- Zinc: Too much zinc supplementation can cause copper deficiency.
B12 and Folate: Always part of the workup.
Clinical pearl: In a bariatric patient with “iron-refractory” anemia, always check copper and zinc. Sometimes the problem isn’t a lack of iron – it’s an imbalance in trace elements.”
