Dietmar Dammerer: How Should We Manage Traumatic Brain Injury When on Anticoagulants?

Dietmar Dammerer, Head of the Department of Orthopedics and Traumatology at the University Hospital Krems, shared on LinkedIn about a recent article he and his colleagues co-authored, published in Archives of Orthopaedic and Trauma Surgery, adding:

How should we manage patients with mild traumatic brain injury who are taking anticoagulants?

Despite the growing number of patients on antithrombotic therapy presenting to emergency departments after head trauma, clinical practice remains far from uniform.

I’m excited to share our newly published nationwide survey examining how Austrian trauma and neurosurgical departments manage patients with mild traumatic brain injury (mTBI) receiving antiplatelet agents, vitamin K antagonists, or direct oral anticoagulants.

Key findings:

  • Nearly all participating departments routinely perform cranial CT scans in anticoagulated patients after mild head injury.
  • Significant regional differences exist regarding hospital admission practices, particularly for patients receiving DOACs and those with hemophilia.
  • Management strategies vary considerably across Austria, highlighting the absence of up-to-date national guidance.

As healthcare systems face increasing patient volumes and resource constraints, standardized, evidence-based protocols are becoming more important than ever.

Our findings underline a clear opportunity: developing contemporary national guidelines that support both patient safety and efficient resource utilization.
