Garima Sharma: Community Clinical Linkages for Better Postpartum Cardiovascular Care
Garima Sharma, Director of Preventive Cardiology and Women’s Cardiovascular health at Inova Health System, shared a post on LinkedIn about a recent article he and his colleagues co-authored, published in AHA Journals Circulation, adding:

In this week of American Heart Association Circulation, Faith Elise Metlock, PhD, RN, Modele Ogunniyi, MD, MPH Kathryn Lindley and I present the clinician’s take on management of Hypertensive Disorders of Pregnancy in the postpartum timeframe by creating community to clinic linkages.
Postpartum monitoring after HDP requires models that extend beyond discharge and link evidence-based clinical care to sustained support in the community.
Studies suggest that community-clinic linked strategies can improve blood pressure monitoring, patient engagement, and transition to ongoing care, but the current landscape remains marked by a paucity of well-studied models.
Expanding this evidence base through larger trials and implementation studies will be essential to building scalable, equitable approaches to postpartum cardiovascular prevention after HDP.
Technology-enabled systems can extend surveillance and coordination through telehealth, remote blood pressure monitoring, text-based support, and digital closed-loop referral platforms that connect women with community resources and track whether those needs are ultimately addressed.
Community-based postpartum programs can broaden access through nurse home visiting, mobile care units, community health workers, doulas, and patient navigators who reinforce discharge education, support home monitoring, identify warning signs, and help women reconnect to obstetric, primary care, or cardio-obstetric follow-up.
Women with HDP need preventive cardiology and closer follow-up for further risk assessment and stratification and optimization of CVH.
